Medical errors are the third leading cause of death in the United States. According to a recent study published in the British medical Journal, the mean rate of death from medical errors is 251,454. Medical errors can occur at the individual level (i.e. an error by an individual nurse or doctor) or at the systemic level (i.e. failures in the hospital or healthcare system that enables an error to occur).
